Since then, Hospice Home has provided care and support to more than 10,000 people aged 5 months to 103 months. With more than 10,000 people served, the impact on our society is huge because every family and friend comes from it. Relatives are also supported by the Visiting Nurse. If your loved one is being cared for in a hospice home, you may have experienced the love and care that the visiting nurse team provides for the whole family.
Hospice Home is a 14-bed hospice inpatient facility that is distinct from a hospital or other healthcare facility. It has a family gathering area designed like a living room in a person’s house, a kitchen, a sunroom, a chapel-like reflection room, and a family conference area. People come to Hospice Home through several different routes. Often, once a patient is in the hospital and is determined to benefit from the 24/7 care provided by hospice professionals to help manage symptoms and provide comfort, they are transferred to a hospice home. A patient can stay at home on the hospice service of a visiting nurse and get into a symptomatic crisis situation. When this happens, the patient is transferred to Hospice Home so that a team of hospice experts can support the patient and family.
Over 20 years, like a home, Hospice Home has seen some wear and tear. We are taking this opportunity to celebrate the anniversary of this community gem and raise funds to repair and restore the building so that we can provide excellent care for the next 20 years and beyond. A referral is required to begin receiving hospital services. Referrals are generally made by the patient’s physician, hospital staff, long-term care/assisted living facility staff, or other health care providers. When you are referred for hospice services, a hospice nurse will assess your condition and discuss your medical, social, spiritual, and personal needs.
For those who need medical intervention that cannot be provided at home, Hospice offers inpatient care in the supportive care unit. Admitted individuals and their families experience patient-centered end-of-life care in a calm, peaceful and intimate setting, allowing patients to spend their final days surrounded by their loved ones. Here you’ll find a patient room with a private bath/shower, couches that provide sleeping space for family members, and two comfortable family rooms that provide a common gathering place outside of the patient room. communicate, share food and enjoy each other’s company.
The palliative care unit is located on the third floor of Randallia Hospital in Menara Negara. For your convenience, a drop-off area is available at Entrance 5, accessed from E.
